Sam, Dean and John were left for dead after the Demon plowed a semi into the Impala. Now, one of the Winchesters hovers between life and death as the surviving family must face an old and powerful adversary.
Moments after the truck slams into the Impala, Sam wakes up to see the driver rip off the door of the Impala. Still possessed by The Demon, the driver taunts Sam before vanishing off, leaving the poor driver bewildered. Sam tries to wake John and Dean up, but neither responds. Even while the paramedics are carrying him into the ambulance, his only thoughts are for his brother and father.
In the hospital, Dean wakes up. He wanders into the hallway and tries to ask a nurse for news on Sam and John. She doesn't hear him or see him. Dean has a feeling he knows what has happened and he rushes back to his room - only to see his body lying broken on the bed, deep in a coma. He's havning some kind of out-of-body experience and no one can hear him, not even Sam. He tries desperately to connect with Sam (who is largely unhurt) or John (who is admitted to the hospital but fully conscious), but there is nothing there. Suddenly, he sees something floating past the door and rushes out to follow it.
He sees a spirit that appears to be killing people. When it eventually heads towards his own unconscious body, he fights it off - his spirit is able to connect physically with it. Meanwhile, Sam and John vow to do anything they can to save Dean. John asks Sam to get some ingredients for a protection spell from Bobby, but Sam discovers that these are actually used for summoning demons. He confronts his dad and demands that he stops trying to fight the demon while Dean's life hangs in the balance. John agrees, but he has another plan up his sleeve.
Dean encounters another spirit, a young girl named Tessa, who's body also appears to be in a coma. They stick together for a while but then Dean heads back to his room where Sam is trying to connect with him through the use of a spirit board. Dean is shocked when he discovers that it works and the brothers manage a sort-of conversation. Dean reveals that he is hunting a reaper. Sam goes to ask John what he can do to help, but John has vanished. More concerned about his brother, Sam retrieves John's journal and the two of them look through it for information on Reapers. As soon as he sees what is written there, Dean dashes off.
He confronts Tessa, telling her that he knows she is a reaper. She admits it and tells Dean that his time is up - he was living on borrowed time anyway. He refuses to go, but Tessa explains that although she cant force him to go with her, if he doesn't, he will remain trapped there as an angry spirit - becoming the very thing he has spent his life fighting.
While Dean and Tessa talk, John summons The Demon. He tells it that he is not there to fight - he wants to make a deal. He offers the Colt to The Demon in exchange for Dean's life. The Demon accepts, but tells John that there is one more thing he wants as well as the gun. Just as Dean is about to make a decision about going along with Tessa, he is returned to his body. He wakes from his coma and the doctors are shocked - all his injuries are healed. Dean cant remember anything about the reaper or how he managed to stay alive.
John comes in. Sam tries to start another argument with him, but John cuts him off. He says he's fed up of fighting and doesn't want to argue with him anymore. Sam agrees and goes to get some coffee. John tells Dean that he has always been proud of him and tells him to watch over Sam. He whispers something else to Dean and walks out.
When Sam returns with the coffee, he see's John on the floor in another room, bleeding. Doctors fight to keep him alive, but at 10:41am, John Winchester is pronounced dead.
